Students may benefit from the School Based Vision Program this summer who are still in need of eye care. In order to provide accessible care to these students, we will be opening our office doors to all CPS students this summer! We will be scheduling eye exams in our South Loop/Pilsen office, by appointment only.
We will need your help to make sure students are aware of this opportunity that's available to them. We have attached flyers to this email with information regarding our summer program. We ask that you distribute this information to your students in any manner that will best work for your parent population (e.g. distributing copies of our flyers, posting on your websites/facebook pages, making robocalls to students homes, etc).
Also, if you have signed consent forms that might have been collected too late to schedule this SY, please let us know before the end of the school year. We will work on collecting those from you so that we are able to schedule your students early next SY.
